I'm still learning, but here's my attempt at the first two lines (it gets hard to make out and part is torn anyway after that):

Admins! Why can't we type Arabic letters on an oud forum?

I can't type the two dots on top of the heh or the two dots under the yeh because the keyboard layout I use for it is Farsi, not Arabic.
I don't speak Arabic, but according to google translate, this means:
Carpentry and modern art
To its owner: Knowledge is tenderness
So... I guess it's not a name, which may provide limited clues as to origin.

That's right,
But in the name is : al'muallem (mu3alem) Hassan : al-Muallem means the crafter/luthier
The small line under the name: crafting and repair of all the instruments (of Tarab) : ancient, modern and classical instruments
The third line : is the address at Casablanca, Morocco
